65.02.4 Application for air traffic service licence

(1)An application for the issuing of an ATS licence shall be made to the Director in the appropriate prescribed form.

 

(2)An application referred to in subregulation (1) shall be accompanied by—
(a) an original or certified copy of identity document of an applicant showing;
(b) proof of age of an applicant if not indicated in the identity document;
(c) appropriate medical certificate issued in terms of Part 67;
(d)appropriate certificate of competency as prescribed in Document SA-CATS 65, which has been signed by a rating assessment examiner;
(e) appropriate fee as prescribed in Part 187; and
(f) two recent passport size photographs of the applicant.
