| (1) | The prescribed forms in these regulations are as follows: |
| (a) | Form A: Registration of a class licence (Regulation 7(1)); |
| (b) | Form B: Registration of a class licence to provide a Community Broadcasting Service and (Regulation 7(2) and to provide a Special Event and Temporary Community Television Broadcasting Services (Regulation 13A); |

| (c) | Form D: Application to amend a class licence (Regulation 8(1)); |
| (d) | Form F: Notice to renew a class licence (Regulation 9(1)); |
| (e) | Form H: Application to transfer a class licence (Regulation 10); |
| (f) | Form K: Notice of surrender of a class licence (Regulation 11)); |
| (g) | Form L: Notice of change of information in respect of a class licence (Regulation 12); |
| (h) | Form M: Licence exemption services and networks (Regulation 13); |
| (i) | Form N: Registration for class licence to provide a Special Event and Temporary Community Television Broadcasting Services (Regulation 13A); and |
| (2) | Forms referred to in regulation 4(1) must be used for any registration, notice or application and may be; |
| (a) | downloaded from the Authority's website; |
| (b) | obtained from the Authority at its offices. |
