33. Certificate of Service

On termination of employment an Employee is entitled to a certificate of service stating:

 

33.1the Employee's full name;

 

33.2the name and address of the Employer;

 

33.3a description of the Bargaining Council by which the Employer's business is covered;

 

33.4the date of the commencement and the date of termination of employment;

 

33.5the title of the job or a brief description of the work for which the Employee was employed at the date of termination;

 

33.6the Employee's remuneration at date of termination; and

 

33.7if the Employee so requests, the reason for termination of employment.
