6. Remuneration paid by applicant

An applicant shall in writing provide the Minister or the board, as the case may be, with all information in respect of any remuneration of whatsoever nature, including but not limited to fees, salaries, commissions, allowances, contributions, bonuses, expenses or reimbursements, irrespective of whether it was described as such or not, which have been paid to directors or members of the applicant, as the case may be, or any of the applicant's employees who are or were members of the applicant's management.
