6. Application for registration as manufacturer, importer or exporter

(1) An application for registration in terms of clause 5 shall be made on the application form that is available from SAGIS.

 

(2) The application form shall be completed on a computer or in ink by a person who is duly authorised and it shall be accompanied by the corroborating documentation as specified in the application form.

 

(3)The application form shall be submitted to SAGIS at any of the addresses furnished on the official application forms.
