The COP should have a title page reflecting at least the following:

 

1.1Name of mine;
1.2The heading of the COP: The provision of personal protective equipment for women in the South African mining industry;
1.3A statement to the effect that the COP was drawn up in accordance with the guideline DMR 16/3/2/5-A2 issued by the CIOM;
1.4The mine's reference number for the COP;
1.5Effective date; and
1.6Revision dates.
