The COP should have a title page reflecting at least the following:

 

1.1Name of mine;
1.2The heading: "Mandatory Code of Practice for the Operation of "Trackless Mobile Machines";
1.3A statement to the effect that the COP was drawn up in accordance with Guideline DMR 16/3/2/2-B2 issued by the CIOM;
1.4The mines reference number for the COP
1.5Effective date; and
1.6Revision dates.
