The title page must include the following:

1.1Name of mine;
1.2The heading; "Mandatory Code of Practice for the Operation of Underground Rail Bound Transport Equipment";
1.3A statement to the effect that the COP was drawn up in accordance with guideline DMR 16/3/2/2-A4 issued by the CIOM;
1.4The mine reference number for the COP;
1.5The effective date; and
1.6Revision dates.
