Part 1 : Registration of persons or facilities

31. Application for registration

(1)A person may apply for registration on an application form provided by the issuing authority, which contains, as a minimum, the information as set out in subregulation (2).

 

(2)An application form contemplated in subregulation (1) must, to the extent applicable, make provision for the following information—
(a)the applicant’s details, including—
(i)the applicant’s name;
(ii)the name of the owner of the property, facility or venture, as the case may be, if the applicant is not the owner;
(iii)identity or passport number;
(iv)telephone number;
(v)cell phone number;
(vi)fax number;
(vii)e-mail address;
(viii)postal address; and
(ix)physical address;
(b)details of the facility including the—
(i)name of the facility;
(ii)kind of facility;
(iii)size of the facility;
(iv)date of establishment of the facility;
(v)physical address where the restricted activities will be carried out; and
(vi)farm name and registration number;
(c)details of the species involved, including—
(i)scientific name (genus, species, and to the extent applicable the subspecies);
(ii)common name (if any);
(iii)quantity per species;
(iv)sex of the specimens;
(v)size or age of the specimens; and
(vi)markings of the specimens.

 

(3)A person applying for registration must pay the applicable processing fee as set out in Annexure 3 to these Regulations.

 

(4)Notwithstanding subregulation (3), payment of the applicable processing fee as set out in Annexure 3 to these Regulations does not apply to organs of state.
