| (1) | An applicant shall make an application for appointment to a post referred to in regulation 2 in writing on a form which corresponds substantially with Form 1 of Schedule A and hand it in to the chief executive officer. |
| (2) | The application referred to in subregulation (1) shall be accompanied by— |
| (a) | a certified copy of the applicant's identity document; |
| (b) | a completed health questionnaire which corresponds substantially with Form 2 of Schedule A; |
| (c) | certified copies of all educational qualifications of the applicant; |
| (d) | certificates of service or, if not available, an affidavit by the applicant in respect of previous periods of service rendered by him or her; |
| (e) | testimonials, if available, from previous employers of the applicant; |
| (f) | an affidavit setting out the applicant's assets and liabilities; and |
| (g) | the names and addresses of two referances. |
| (3) | The chief executive officer shall submit the application referred to in subregulation (1) to the Commission for a recommendation regarding the suitability of the applicant. |
| (4) | The chief executive officer shall implement the Commission's recommendation referred to in subregulation (3). |
