| (1) | An appeal or review must be made by an applicant on form AARTO 10A. |
| (2) | The form contemplated in subregulation (1) must— |
| (a) | include copies of additional supporting documents relating to the review, where applicable; |
| (b) | be accompanied by the fee specified in Schedule 2, paragraph 5.5; and |
| (c) | be submitted in accordance with regulation 35(1). |
| (3) | In the event that form AARTO 10A is not completed and signed, the designated employee must notify the applicant to resubmit a comprehensively completed and signed form AARTO 10A on form AARTO 10Aa. |
| (4) | If the applicant fails to comply with the directions contemplated in subregulation (2), the designated employee may inform the applicant on form AARTO 10Ab of his or her failure to comply and that such appeal or review is regarded as unsuccessful. |
