35. Certificate of Service

On termination of employment an Employee is entitled to a certificate of service stating:

 

35.1the Employee's full name,

 

35.2the name and address of the Employer,

 

35.3a description of the Bargaining Council by which the Employer's business is covered,

 

35.4the date of the commencement and the date of termination of employment,

 

35.5the title of the job or a brief description of the work for which the Employee was employed at the date of termination,

 

35.6the Employee's remuneration at date of termination and

 

35.7if the Employee so requests, the reason for termination of employment.
